FREE Phonological and Phonemic Awareness Assessment

Disclaimer: This assessment is only a snap-shot of your learners’ abilities. You will want to observe learners as they play with sounds in words and even try to spell words to get a better picture of what they understand.

This assessment is meant to be administered one learner at a time. It works best if your learner waits at least four weeks in between assessing the same skill again. This keeps the assessment fresh and more accurate.

The skills on this assessment become gradually more difficult. Skills 1-6 are at the basic phonological level, as they ask learners to focus on whole words (such as rhyming words) or larger word chunks (syllables). Skills 7-14 are at the phoneme level. These skills ask learners to focus on the individual sounds (phonemes) they hear in words.

Fourteen skills are included with eight tasks for each skill. If a learner misses MORE THAN TWO tasks in any skill section, this may indicate a skill the learner still needs to work on.

For the most part, this assessment can be done without the visual student pages. After all, it has been said that phonological awareness and phonemic awareness tasks can be done “with your eyes shut.” The student pages were included to make the skills easier for visual learners or for learners who might struggle to retain all the information without some visual reminders. If you choose not to use the visual student pages, some of the skill prompts may need to be modified.

All but two of the skills have an accompanying section of visuals on the student pages.

Each time you assess the same learner, it may be helpful to use a different colored pen. Check marks under “Check if correct” and dates for “Date Completed” can be written in the new color each time you assess. This way, you can quickly see when the learner has mastered certain skills.

Optional: Provide a blank piece of colored paper (cardstock or construction paper) so that learners can cover the skills they are not doing only to reveal the one they are working on.
